v1.3 fixes the Notice: undefined offset at line 331 messages. Upgrade if you get these notices. The messages are due to the optional Method title being omitted from the rate line. e.g.

0|0|1
700|10|1

Workaround

Append a separator character after the country group number

0|0|1|
700|10|1|

Alternatively, define WP_DEBUG as false.
